PDS and Traditional Services Offered

At 2nd Home, we offer Case Management services under Kentucky’s Home and Community-Based (HCB) Waiver. This waiver helps eligible individuals receive the support they need to remain safely in their homes and communities - and we’re here to help guide the way.

There are two models of care under the HCB waiver:

• Traditional: Services are provided through approved agencies.

• Participant Directed Services (PDS): Participants or their representatives take the lead in hiring and managing their own caregivers.

2nd Home is approved to provide case management for both Traditional and PDS models, and we’ll help you explore whichever option is right for you.

Once you’re approved for the waiver, your Case Manager will be your first point of contact. We’ll meet with you and your chosen care team in your home to create a personalized care plan that reflects your goals, needs, and preferences. We’ll guide you step-by-step through accessing services, whether you’re using Traditional care or directing your own through PDS.

Your Case Manager will stay in touch each month to ensure services are working well and to make adjustments as needed. We also assist with annual renewals and ongoing support throughout the year.

If you or a loved one doesn’t speak English, language assistance is available. We are proud to support participants from many cultural backgrounds and will make sure you can understand and take part in your care every step of the way.
